Furniture Request Program

If you are in need of furniture, we may be able to help. All clients must be referred to us by one of our referring organizations. To find out if you/your family are eligible, contact one of these agencies. Once you have a case manager they may download the furniture request form. They may send it us, filled out, by emailing referralsmahfct@gmail.com or by faxing (203) 527-7136.

Please note that you cannot make an appointment for yourself. Make a Home Foundation only accepts clients through a case manager or other independents.
